Abstract "You are working at a community Emergency Department when a nurse tells you that there is an 18-year-old somnolent female with concern for acetaminophen (APAP) overdose. Vitals in triage show a blood pressure of 90/60 mmHg, heart rate of 130 bpm, respiratory rate of 23 rpm, and an SpO2 of 97% on room air. The patient is accompanied by her mother who tells you her symptoms began within the last hour. She appears unwell, seems confused, and has vomited multiple times. The mother states that her daughter has been depressed recently. She went in to check on her in her room approximately 1 hour ago and found her minimally responsive on her bed with a large 1,000 tablet bottle of 500mg APAP lying on the floor next to her. The patient did have some vomit on her shirt that contained pill fragments. After obtaining an appropriate initial work-up the patient's labs show an elevated acetaminophen level, an anion gap metabolic acidosis, an elevated lactic acid and normal AST/ALT levels"-- Provided by publisher.
